The first visit will include:

• Consultation with the dentist

• Diagnostic digital x-rays as needed

• Comprehensive dental exam and screening for cavities

• Occlusal analysis to look for bite problems

• Periodontal screening

• Oral cancer screening

Please bring with you:

• Recent dental x-rays if possible

• Any pertinent medical history or information

• Adult new patient forms AND HIPAA forms

• Insurance cards

• Form of personal identification, such as a drivers license
